Breast implant exchange replaces existing implants with new ones. Patients choose exchange because implants are aging, have ruptured, no longer suit their goals, or because they want a different size, shape, or implant type. Because implants are not lifetime devices, exchange is a normal part of long-term breast surgery.
There are two main approaches. A simple exchange reuses the existing pocket, swapping the implant directly. A new-pocket exchange creates a fresh, better-positioned space when the original pocket has stretched, the implant has shifted, or improved positioning is needed. The right approach depends on the condition of your tissue and the original surgery.
Exchange can be combined with a capsule adjustment, a change in placement, or a lift if sagging has developed. The goal is to refresh and improve your result safely. All surgery carries risks, and results vary.
Exchange alone does not correct significant sagging, which may need a lift, and does not remove a thickened capsule unless a capsule procedure is included. It cannot make implants permanent.
You will receive instructions on medication, supplements, smoking, and fasting, and imaging may be arranged to assess the current implants and capsule. The exchange approach, simple or new pocket, and any added procedures are planned together.
The existing implant is removed through the original incision where possible, the pocket is assessed, and the new implant is placed in the same or a freshly created pocket. Capsule work or a lift is performed at the same time if planned.
Recovery is often slightly faster than the original surgery when the same pocket is used. Swelling and tenderness ease over the first weeks. A support bra is worn as advised, and strenuous activity is reintroduced gradually. The result settles over a few months.
A refreshed, updated result with new implants is achievable. Exchange cannot make implants last indefinitely, cannot lift heavy sagging alone, and cannot guarantee a result identical to the original surgery, since tissue changes over time.
