Upper eyelid surgery

Upper eyelid surgery, or upper blepharoplasty, removes excess skin and, where needed, a small amount of fat from the upper eyelids. With age the upper lid skin stretches and can hood over the lashes, making the eyes look heavy or tired and sometimes affecting the field of vision.

A fine incision is hidden in the natural crease of the eyelid, so the scar is well concealed once healed. The result opens the eye and refreshes the upper face without changing its character.

It is one of the most rewarding facial procedures, with a relatively quick recovery. It does not lift the brow or treat the lower lids, which are separate procedures. All surgery carries risks, and results vary.

Before the procedure

You will receive instructions on medication, supplements, smoking, and fasting. The brow position is assessed as well as the lids, since a low brow can mimic eyelid hooding and change the plan.

During the procedure

Excess upper-lid skin, and a small amount of fat if needed, is removed through an incision hidden in the natural eyelid crease, then closed with fine sutures.

After the procedure

Swelling and bruising around the eyes settle over one to two weeks, helped by cool compresses and keeping the head elevated. Sutures are removed within about a week, and most patients return to social activity soon after.

Results: what it can and cannot achieve

More open, refreshed, less tired-looking eyes are achievable, with a well-hidden scar. It cannot raise the brow or treat the lower lids, and it does not stop further aging.

Possible risks and complications

Upper eyelid surgery is generally safe, but all surgery carries risks. Possible risks include:

• Bruising and swelling
• Infection
• Temporary dry or irritated eyes
• Asymmetry between the lids
• Visible scarring, usually well hidden
• Difficulty fully closing the eyes, usually temporary
• Rarely, changes in vision

Existing dry eye or eye conditions should be discussed, since surgery can affect them. Smoking impairs healing.

Upper eyelid surgery may suit patients with heavy or hooded upper lids. It is especially suitable for patients who:

• Have excess skin or hooding on the upper lids
• Look tired or heavy around the eyes
• Want a refreshed, more open eye
• Are in good general health with realistic expectations and do not smoke or can stop before and after surgery

It may not suit patients whose hooding is mainly from a low brow, who may need a brow lift, or those with untreated eye conditions. Your surgeon assesses the brow and lids together.
